To obtain the LGV / HGV Licence
The first step is to complete DVLA form D2. This is the application form for a provisional licence.
You will also need a medical examination with a doctor who will complete DVLA form D4.
You then send off both parts of your Driving Licence together with the D2 and D4 forms to the DVLA in Swansea. The D2 and D4 forms are available to order from DVLA or we can supply if required.
